Family Nurse Practitioner - Primary Care
We are actively seeking a dedicated and compassionate Family Nurse Practitioner to join our primary care practices in the vibrant Finger Lakes region of New York, including Dundee, Seneca Falls, and Penn Yan. Compensation: $115,000.00 - $135,000.00
Essential Duties and Responsibilities:
- Provide comprehensive and continuous primary care to a diverse patient population.
- Develop and follow patient care plans and collaborate with physicians and other health professionals as needed.
- Educate patients and families on health care issues and preventative measures.
- Manage chronic conditions, and provide interpretation of test results.
- Participate in community health initiatives as applicable.
- Maintain detailed and accurate patient records.
- Engage in ongoing professional development and remain current with advancements in the field.
Minimum Position Qualifications:
- Current Board NP Certification.
- Active New York State License.
- Graduate of an accredited Nurse Practitioner or Physician Assistant program.
- Demonstrate clinical competence and knowledge of professional nursing theory, practice, and medical care.
- Ability to work both independently and collaboratively as part of a multidisciplinary team.
- Strong communication skills and the ability to build effective relationships with patients and families.
Preferred Qualifications:
- Experience in primary care, family practice, or a similar setting is highly preferred.
- Knowledge of electronic medical record (EMR) systems.
- Skills in a broad range of medical procedures and diagnostics.
- Excellent decision-making and problem-solving skills.
- Prior experience with rural health care settings.
Skills:
- Expertise in creating and implementing effective care plans.
- Strong diagnostic skills and proficiency in using medical tools and equipment.
- Capability in managing time effectively to see patients and complete accurate documentation.
- Excellence in patient and family education to promote health and manage conditions.
- Ability to respond effectively to emergencies and use clinical judgment in acute situations.
