Family Nurse Practitioner - Primary Care

We are actively seeking a dedicated and compassionate Family Nurse Practitioner to join our primary care practices in the vibrant Finger Lakes region of New York, including Dundee, Seneca Falls, and Penn Yan. Compensation: $115,000.00 - $135,000.00

Essential Duties and Responsibilities:

Provide comprehensive and continuous primary care to a diverse patient population.
Develop and follow patient care plans and collaborate with physicians and other health professionals as needed.
Educate patients and families on health care issues and preventative measures.
Manage chronic conditions, and provide interpretation of test results.
Participate in community health initiatives as applicable.
Maintain detailed and accurate patient records.
Engage in ongoing professional development and remain current with advancements in the field.

Minimum Position Qualifications:

Current Board NP Certification.
Active New York State License.
Graduate of an accredited Nurse Practitioner or Physician Assistant program.
Demonstrate clinical competence and knowledge of professional nursing theory, practice, and medical care.
Ability to work both independently and collaboratively as part of a multidisciplinary team.
Strong communication skills and the ability to build effective relationships with patients and families.

Preferred Qualifications:

Experience in primary care, family practice, or a similar setting is highly preferred.
Knowledge of electronic medical record (EMR) systems.
Skills in a broad range of medical procedures and diagnostics.
Excellent decision-making and problem-solving skills.
Prior experience with rural health care settings.

Skills:

Expertise in creating and implementing effective care plans.
Strong diagnostic skills and proficiency in using medical tools and equipment.
Capability in managing time effectively to see patients and complete accurate documentation.
Excellence in patient and family education to promote health and manage conditions.
Ability to respond effectively to emergencies and use clinical judgment in acute situations.
