Join Cottage Health as the Clinical Counselor, per diem. The Clinical Counselor is responsible for facilitating group therapy, program referrals, assessments, and admissions, and coordinates with outpatient providers and patient supports networks for safe complete discharge planning. Major accountabilities include:
- Serves as liaison to community agencies and therapists; Communicates to medical staff and treatment team involved in patient care and Coordinates treatment planning sessions.
- Processes referrals, evaluations, and admissions to internal and external programs.
- In conjunction with the appropriate team members, schedules activities and leads a variety of therapeutic activity groups. Provides individual psychotherapeutic interventions necessary to promote optimal health. Makes required documentation in patient records.
- Assists with promoting program services to the community and serves as liaison to local agencies, programs, and clinicians. May serve as a clinical supervisor to students and associates.
- Assists with the development of quality assurance and improvement standards and audits charts to assure charting and procedures on all patients is complete.
- Assists with collection of statistical data and initiation, distribution, and examination of program surveys.
- Assists with development of policies and procedures for intake, assessment, programming, treatment planning, charting, discharge planning, special treatment procedures and maintenance of a therapeutic environment in a manner compliant with The Joint Commission, HIPAA and Title 9 Regulations (DHCS).
- Participates in treatment planning and review conferences. Provides input to the treatment planning process. Identifies planning problems that need early intervention. Incorporates specific discharge plans into plan of care and revises plan of care as treatment progresses.

LEVEL OF EDUCATION
Minimum: Master's degree in psychology, counseling, and/or social work
YEARS OF RELATED WORK EXPERIENCE
Preferred: 1 year recent acute psychiatric experience.
