Serenity Hospice is looking to add a dedicated and organized Hospice RN Team Lead to our Central team. The Hospice RN Team Lead is responsible for providing leadership and direction to the designated team of nurses to ensure the highest standards of patient care.
The Team Lead is responsible for developing and implementing patient care plans, assessing patient needs, and ensuring that all care is delivered in accordance with established protocols and best practices.
Responsibilities:
- Participates in the coordination and planning of the delivery of quality, cost-effective hospice care.
- Evaluates patient services provided by the interdisciplinary team.
- Maintains working knowledge of hospice state licensure, federal and accreditation requirements.
- Monitors services and documentation to meet hospice policies and procedures, state, federal, and accreditation guidelines.
- Serves as a liaison to the Administrator regarding clinical challenges and progress in patient care.
- Supervises and participates in the provision of care provided, orientation, scheduling, education, and support of all caregivers on the hospice interdisciplinary team, including any contracted staff.
- Be available on-call if needed. Provide support for on-call staff.
- Schedules and runs meetings for the interdisciplinary team (IDT).
- Develops physician relationships to familiarize them with hospice care.
- Acts as a liaison between IDT members, patients, and families.
- Contributes to effective patient and customer relationships by assisting patients, families, physicians, and staff to resolve expressed concerns.
- Facilitates interdepartmental communications with co-workers and staff.
- Performs other duties as needed.
Requirements:
- Current NJ RN License in good standing
- 2 Years of Hospice experience required
- Prior progressive management responsibilities
- Strong organizational skills.
- Valid Drivers License with Auto Insurance.
- Reliable transportation and willing to travel.
- Proficient in Microsoft Word and Excel.
- Strong computer skills and ability to learn new software.
- Excellent communication skills, written and verbal.
- Ability to multitask projects and assignments.
