We are seeking a compassionate and dedicated Hospice Nurse to join our team. As a Hospice Nurse, you will be responsible for providing end-of-life care and support to patients in hospice settings. Your primary focus will be on ensuring the comfort and quality of life for patients and their families during this sensitive time.
The Registered Nurse Case Manager (RNCM) works under the general direction of the Patient Care Manager (PCM), The RNCM serves as a member of the Interdisciplinary Team (IDT) and coordinates patient care consistent with the Plan of Care (POC) and is responsible for provision of services consistent with the mission, vision, and goals of the agency, quality outcomes and regulatory standards
*Duties and Responsibilities*:
· Assumes primary responsibility for assigned patients and performs all phases of the nursing process
· Provide skilled nursing care to hospice patients in accordance with the established care plan
· Communicates with attending physicians, Gracious Care Hospice staff members, and other agencies as necessary to coordinate optimal care and use of resources for patients/caregivers
· Conduct assessments, monitor patient conditions, assess medications needs
· Collaborate with interdisciplinary teams to develop and implement individualized care plans
· Offer emotional support and guidance to patients and their families
· Educates patients/caregivers regarding hospice philosophy, disease progression, physical components of care, symptom management and end-of-life care options, stages of dying, and other aspects of the POC as necessary.
· Coordinate with healthcare providers to ensure continuity of care
· Maintain accurate patient records and documentation in accordance with Gracious Care Hospice policies and procedures
· Seeks input from IDT members regarding the patient POC to obtain additional knowledge and support
· Regularly attends and participates in IDT meetings to ensure that the POC reflects current patient care needs
· Assesses need for Caregiver and/or Certified Nursing Assistant (CNA) services
· Conducts caregiver and/or CNA supervisory visits per regulatory standards.
· Assess the need for and makes revisions to the CNA care plan.
· Provides on-call coverage as scheduled.
· Maintains annual clinical competencies.
· Identifies patients who may potentially revoke or need to be discharged.
· Models prompt and regular attendance.
· Exhibits cost effective use of time, mileage and supplies.
· Promotes and maintains respect for rights, privacy, and confidentiality of others.
· Respects patients and staff confidentiality always.
· Attends staff meetings, mandatory in-services, and other meetings as appropriate or upon request.
· Participates in committees, events as requested.
· Assumes responsibility for own personal and professional growth including state licensing/certification requirements.
· Understands and exemplifies hospice philosophy, values, beliefs and team concepts in all job-related roles.
· Abides by dress code.
· Other duties assigned.
Qualifications:
- Bachelor's degree in nursing (BSN) required
- Active RN license in the state of Oregon
- Minimum 2 years of experience as a RN preferred
- Previous experience in hospice palliative medicine
- Knowledge of Memory Care, Residential Care Facility, Assisted Living, Adult Family Home.
- Strong communication and interpersonal skills
- Compassionate attitude and ability to provide emotional support
- Valid driver’s license, car insurance and reliable car
