About Twin Cities Physicians: Established in 2016, Twin Cities Physicians now serves over 4,500 residents within 200+ Senior Living Communities in the State of Minnesota. We take a coordinated and collaborative approach to keeping residents healthier longer and safer in their communities, helping to avoid unnecessary ER and hospital visits. Our goal is to alleviate some of the burden and stress senior communities and families feel trying to coordinate needed medical services for residents and their loved ones.
Position Summary: Twin Cities Physicians is seeking a Physician Assistant to join our care team in Golden Valley! With Twin Cities Physicians, you will be providing the best care for individuals within all types of senior living communities. We work with many communities across the Twin Cities Metro area, bringing primary care right to our patients on a weekly basis!
Responsibilities:
- Drive to Senior Living Communities to provide on-site primary care to residents.
- Facilitate the patient's plan of care: chronic care management, advanced treatment, and end-of-life planning.
- Coordinate with the interdisciplinary team, reconcile medications, and order labs.
- Communicate clearly and regularly with Physician, office support staff, community staff, and patients' families.
Qualifications:
- Current registration as a Physician Assistant / PA by the MN Board of Medical Practice.
- Strong portfolio in Geriatrics, Primary Care, or Internal Medicine.
- Passion for serving the Older Adult Population.
- Strong interpersonal communication skills.
- Valid driver’s license and current auto insurance.
