Full Time Nurse Practitioner - Stroke Program - Modesto area
Central Valley hospital

Experience needed: MSN, Nurse Practitioner, Must be CA licensed RN/NP, NP with stroke experience preferred however the team is willing to consider a new grad NP with a strong ICU, neuro, and ED experience as a RN. DEA, BLS, ACLS

Stroke Program Nurse Navigator (Nurse Practitioner) provides expert nursing consultation and practice oversight, functioning as a liaison between medical staff, nursing staff, family members and support services.
Coordinates multidisciplinary care based on evidence based practice. Incorporates the patient experience in all aspects of patient care.
You are responsible for providing stroke education, delivery and oversight of patient care, including post discharge follow up.
Works in collaboration with Stroke Program key stakeholders to meet stroke program certification requirements, including review of data and performance improvement projects.
Reports to the Stroke Program Coordinator
