Organizational Summary:
Living Well Disability Services delivers exceptional services that transform the lives of people impacted by intellectual, developmental and physical disabilities. Services are available to people in their own home, their family’s home or in one of our 36 group homes throughout the 10-county region. We offer a broad spectrum of people with intellectual, developmental, and physical disabilities such as cerebral palsy, Down syndrome, multiple sclerosis, autism, and brain injury. These may be complicated by a variety of chronic health conditions. Primary Function
Provide assessment of health status, direct nursing and health support to people receiving services, and provide direction to staff in addressing health concerns and safety to assure accurate intervention and follow up.
Essential Job Functions
- Provide nursing and health care support to people receiving services within the scope of nursing practice.
- Coordinate the development, implementation, evaluation and documentation of health care plans in accordance with interdisciplinary team recommendations.
- Coordinate internal and external health consultation related services to meet individual outcomes.
- Consult with internal and external health care providers to coordinate services to meet individual outcomes.
- Provide direction and supervision of unlicensed staff regarding health related issues within the scope of nursing practice.
- Receive clinical consultation from a nurse consultant. Communicate nursing concerns to the nurse consultant.
- Provide consistent, accurate and effective verbal and written communication with staff, organizational support services, families, and outside professionals to assure continuity of care and to fully meet each person’s specific health care needs in a professional and confidential manner.
- Communicate respectfully and confidentially with internal and external colleagues. Communicate in accordance with chain of command.
- Work directly and effectively with people receiving services, staff and others as needed to provide optimal health care to each person.
- Work to assure that a safe, healthy living and working environment is provided.
- Continue to further clinical knowledge and skills by completing continuing education activities.
- Safely lift and transfer at least 50 lb., unassisted, up to 10 times per hour.
- Other duties as assigned.
Qualifications

- Demonstrated ability to identify, develop and communicate an integrated plan to address individual health needs.
- Demonstrated proficiency in managing health services for people who have disabilities and other related conditions including the ability to effectively teach, train and mentor staff to provide effective healthcare services to people.
- Demonstrated thorough knowledge and understanding of current health related practices, issues and regulations relative to people with disabilities and related conditions.
- Demonstrated ability to work independently and as an effective member of multiple teams.
- Demonstrated ability to work effectively with individuals, staff, families and legal representatives.
- Demonstrate proficient, effective verbal and written communication.
- Demonstrate proficiency in record keeping procedures.
- Valid driver’s license and approved as a driver for Living Well Disability Services.
- License as a LPN or RN in the State of Minnesota. In the event the Minnesota Board of Nursing limits scope of nursing practice, supervisor must be notified immediately.
Typical Work Environment
Employees will work in an indoor setting. However, while performing the duties of this job the employee may work in outside weather conditions. The noise level in the work environment is usually moderate, and sometimes loud.
Areas of Responsibility/Accountability
Planning: Follow medication and health care policies and procedures as determined by the organization. Perform ongoing evaluation of each person’s status to assure appropriate health care is being provided. Assist in the development of health care profiles and nursing/health care assessments as needed.
Organization: Effectively train and mentor staff to develop the skills necessary to assist and support people to achieve desired health care outcomes including the use of medications and treatments. Ensure employees understand the specifics of each individual’s health care plan and needs, providing additional training as needed.
Communication: Work and communicate effectively and cooperatively with a variety of people and organizational teams.
Teamwork: Work as a team member to support all people to help them maintain the health they need to access their communities, act on potential and meet their desired outcomes.
