This full-time Clinical Case Manager position in our Community Support Program (CSP) offers a wide array of services to chronically mentally ill adults under the supervision of the CSP Clinical Coordinator. This position is MA Billable.
The majority of clinical services are provided in the client's home or community, with travel required throughout Green County. Typical hours of coverage include M-F from 8:00 am to 4:30 pm, but also include: one late shift (10:30 am to 7:00 pm) every other week, nine late Friday shifts (10:30 am to 7:00 pm) per year, one holiday shift (12:45 pm to 6:00 pm) per year, three weekends (Saturday and Sunday 10:00 am to 7:00 pm) per year, and assigned monthly on-call.
Duties include:
- Psychiatric assessment, treatment, and clinical case management.
- Provide psychotherapeutic, supportive, educative, and psychosocial services to an assigned number of clients or client groups in the areas of psychiatric symptomatology, interpersonal/social opportunities, general medical/dental health, and vocational rehabilitation.
- Develop and coordinate resources to assure basic needs of clients are met.
- Deliver and observe client's medications as needed.
- Document client progress in client record.
- Provide representative payee services for assigned clients, complete necessary paperwork, and consult with outside agencies to maintain client benefits.
- Perform mental status and suicide assessments of CSP clients and provide crisis intervention services.
- Attend and participate in daily CSP interdisciplinary team meetings.
- Develop, implement, evaluate, and revise overall treatment goals, plans, crisis plans, and functional screens for assigned clients.
- Consult with community agencies and families to maintain involvement and coordination of the treatment process.
Requirements:
Master’s degree in social work, clinical psychology, behavioral science, or psychiatric mental health nursing, or equivalent requirements and experience providing services to the chronically mentally ill population.
Possession of a valid Driver’s License and access to personal transportation is required.
Compensation and Benefits:
Clinical Case Manager starting pay is $29.19/hour ($60,715.20 annually).
