Job Description
The LPN is responsible to assure the individual receives quality health care services to persons with disabilities within the prevailing standards or current practice of community-based health care. The multi-site nurse maintains continuation of the nursing process for each individual authorized to receive nursing services under the supervision of the registered nurse.
The LPN assists the supervisor and the nurse supervisor in training and orientation of staff, provides leadership and guidance to staff in the supervisor’s absence, coordinates the health services of individuals under the supervision (as defined by the Minnesota Nurse Practice Act) of the nurse (designated nurse and/or health services coordinator), performs specifically delegated medical and nursing actions, as determined by the physician and/or nurse.
Qualifications:
- Must be a licensed practical nurse, currently licensed in the State of Minnesota, preferably with experience in service for persons with disabilities or long-term care.
- Must be responsible, reliable, flexible and able to carry out job functions without direct on-site oversight by the supervisor and/or the registered nurse.
- Must have basic computer skills.
- Must have a valid driver's license with an acceptable driving record.
- Current auto liability insurance.
- Reliable transportation.
