Responsibilities
The Advanced Practice Registered Nurse (APRN) - dual certified in Psychiatric & Family Medicine is an essential team member responsible for providing and ensuring the highest standards of safety and continuity of care. This role includes direct services with children, adolescents, or young adults and their families including but not limited to evaluation/assessment; medication therapies; psychotherapy and counseling; parent/client education; record keeping; and collaboration with other professionals and community services.
Essential Duties:
- Functions as an effective member of the mental health team; oversees Psychiatric and Medical care of assigned clients and manages a regular caseload.
- Evaluates and manages assigned clients through direct care encounters weekly, and as needed.
- Ensures weekly follow-up appointments are performed and documented timely (within 24 hours of appointment).
- Timely completion of documentation including assessments, treatment plans, progress and group notes, discharge summary, according to established standards.
- Ensures that a Psychiatric Evaluation is performed and documented timely.
- Provides continuity of care by connecting to other providers upon admission to obtain history and collateral information.
- Attends and participates in required treatment teams, staff meetings, and training.
- Shares in the on-call duty responsibilities (Residential programs only).
- Completes discharge recommendation assessments and connections with providers upon discharge.
- Assists with providing cross-coverage to Psychiatry team when needed.
- Maintains excellent communication with other members of the clinical team, outpatient providers, and family members.
- Performs other duties as assigned.
Qualifications
- Master's Degree in nursing from an accredited institution and Board Certification in Psychiatric & Family Medicine.
- Licensed as a Nurse Practitioner in the state in which you are applying.
- Current DEA license.
- Minimum two (2) years of experience working with adolescents or young adults.
- Experience working with clients in a residential or psychiatric treatment environment preferred.
