Empyrean Hospice hires Patient Services Coordinators (Licensed Practical/Vocational Nurses) to coordinate and assist the Clinical Director in the scheduling of patient visits and maintaining the flow of clerical functions in the office.
Salary or hourly rate listed is the base for this role; increase commensurate with experience.
Position Overview – Licensed Practical/Vocational Nurse:
- Represent Empyrean Hospice with the utmost professionalism and compassion
- Schedule field staff visits and daily review of field staff productivity; create and maintain patient files records utilizing the Management Information System
- Assist the Executive Director/Administrator and maintain the files, supplies, and general office condition
- Handle all correspondence and word processing for the hospice organization including telephone inquiries and call routing
- Ensures doctor's order forms/related forms are current and in compliance with Medicare and Medicaid regulations; coordinates required signatures, orders, and communication logs; maintains forms in patient records
- Support patient care through completing patient visits as needed; under the supervision of a registered nurse and as defined in State Nurse Practice Act, including specialized duties related to end of life care
- Understand the philosophy of hospice and the needs of the terminally ill

Position Qualifications
- Professionalism, compassion, empathy, and a desire to help others
- Graduate of an accredited practical nurse or vocational nursing program
- Currently licensed as an LPN/LVN in the State and complies with accepted professional standards and practice
- Minimum of two (2) years' experience preferably in hospice or health care, or graduation from a one (1) or two (2) year business college preferred
- Experience with HomeCare HomeBase Electronic Medical Record preferred
