ResponsibilitiesThe Dietitian - Diabetes Educator serves as a provider of diabetes care and education within the Norton Healthcare System. Provides direct patient care and support, including evaluation and teaching of use for insulin and anti-hyperglycemic agents, and delivery tools. Manages patients' routine diabetes care through assessment of use of advanced diabetes technologies (including glucose meters, interstitial glucose sensors, and subcutaneous insulin infusion pumps). Delivers comprehensive diabetes care and education for nutrition, basic diabetes management, acute/chronic complication prevention, lifestyle and behavior modification, while assessing comprehension, psychosocial factors, and social determinants of health. Functions as part of multidisciplinary team utilizing age-appropriate principles of growth and development for patients aged infant to geriatric. Participates as needed with organizational departments/committees and partner with external programs to provide robust health and wellness options to patients served. Administers comprehensive diabetes care through on site programs/classes to deliver nutrition and relevant health and wellness education to patients and their families in the ambulatory, emergency department, and hospital-based settings via face-to-face or remote electronic modalities.
QualificationsRequired:
- One year in specialty area
- Bachelor Degree
- Dietitian (State) KY and IN
- Registered Dietitian Nutritionist (National)
Desired:
- Master Degree
- Certified Diabetes Educator (National) OR Licensed Diabetes Educator (State) OR Diabetes Care and Education Specialist OR One of: CDE or CDCES Must obtain within two and a half years of hire OR Certified Diabetes Care and Education Specialist
