Connected Health Care is seeking a dedicated and experienced Registered Nurse (RN) for a local travel contract position in Home Health and Hospice near Santa Fe, NM. This is a 13-week contract with weekly compensation up to $2,680 based on experience. Join our team and provide exceptional care in a supportive and dynamic environment.
Job Description:- Deliver comprehensive nursing care to patients in their homes and hospice settings.
- Conduct patient assessments, develop care plans, and monitor patient progress.
- Administer medications and treatments as prescribed.
- Provide patient and family education on disease management and end-of-life care.
- Utilize WellSky/Kinser software for documentation and care planning.
- Coordinate with other healthcare professionals to ensure cohesive patient care.
- Maintain accurate and detailed patient records.
Education and Certification Requirements:- Active RN license in New Mexico.
- Home Health & Hospice experience required.
- Proficiency with WellSky/Kinser software.
- Current BLS certification.
