Hospice Nurse Case Manager - Full Time
We are looking for a dedicated and skilled Hospice Nurse Case Manager to join our team. This vital role is focused on providing comprehensive care and support to terminally ill patients. As a Hospice Nurse Case Manager, you will play a crucial role in overseeing the palliative care needs of our patients, ensuring they receive the highest quality physical, emotional, and spiritual support. Your responsibilities will include coordinating care plans, managing a team of healthcare professionals, and offering emotional and practical support to patients and their families. Essential Duties and Responsibilities:
- Oversee and coordinate the daily care of hospice patients.
- Manage scheduling of patient visits based on individual needs and ensure all medical orders are followed.
- Collaborate with physicians and other healthcare professionals to adjust care plans as needed.
- Supervise and guide LPNs, LVNs, and Hospice Aides, ensuring all protocols and quality standards are met.
- Administer medications as prescribed and manage any side effects or complications.
- Provide emotional, social, and spiritual support to patients and their families, helping them to cope with end-of-life issues.
- Document patient care services by charting in patient and department records.
- Ensure compliance with federal, state, and local legal and certification requirements.
- Educate patients' families and caregivers on recognizing potential symptoms and managing care needs safely and effectively.
Minimum Position Qualifications:
- Valid Registered Nurse license required.
- Minimum of one year of nursing experience.
- Proven experience in hospice or palliative care settings.
- Ability to serve on-call as needed.
- Access to reliable transportation and a valid driver’s license.
- Excellent organizational and communication skills.
Preferred Qualifications:
- Associate’s degree in Nursing or related field, preferred.
- Advanced certification in hospice and palliative care nursing (CHPN) a plus.
- Prior supervisory experience in a healthcare setting.
Skills:
- Strong clinical skills with an emphasis on pain and symptom management.
- Exceptional interpersonal and empathy skills.
- Proficient in using healthcare software and systems for patient management.
- Ability to lead and motivate a diverse team of healthcare professionals.
- Expertise in crisis intervention and the ability to handle complex situations with sensitivity and professionalism.
