Primary Care at Beaverton is located in Beaverton, Oregon. It is a multi-specialty primary care clinic consisting of Internal and Family Medicine Providers who provide healthcare that encompasses the entire life cycle, from birth through pediatric, adult, and geriatric patients. Specialty services provided at Beaverton include Sports Medicine.
Function/Duties of Position
The Primary Care Clinical Pharmacist works independently, at the highest level of clinical practice within OHSU Primary Care Clinics. They are an integral part of the interdisciplinary healthcare team. The Pharmacist practices under collaborative drug therapy management (CDTM) agreements to initiate, modify or discontinue medication therapy for specific disease states (diabetes, hypertension, COPD, smoking cessation, etc.) and as a consultant for intensive medication therapy management services within their clinic(s). Responsibilities may include the provision of both direct and indirect patient care, professional consultations, drug information services, and other clinical pharmacy services.
The Primary Care Clinical Pharmacist duties include, but are not limited to:
- Promoting the appropriate economical, safe, and effective use of medication therapy.
- Maintaining collaborative and comprehensive medication therapy management services, including physical assessment, ordering lab and diagnostic tests necessary to manage medication therapy, referring patients to appropriate healthcare providers as needed, performing patient triage functions, and independently initiating, adjusting, and discontinuing medications.
- Delivering direct care to patients through various modalities, including face-to-face care, telephone, clinical video teleconferencing (when available), shared medical appointments, group education classes, and/or secured messaging interactions.
- Documenting encounters and clinical decisions in electronic records.
- Responding to requests sent by other members of the healthcare team.
- Maintaining a collaborative management relationship with the patient’s care team.
- Assisting providers by approving refill requests as outlined by the Pharmacist refill protocol.
- Providing professional consultative services related to other healthcare providers, including drug information services, formulary management, and medication recommendations.
- Functioning independently in day-to-day clinical and administrative duties associated with the position, using a high degree of professional judgment to solve complex problems that routinely arise throughout the workday.
Required Qualifications
- A current Pharmacist’s license issued by the Oregon State Board of Pharmacy.
- Experience in community pharmacy practice.
- Knowledge of drug therapy in patients as described in Position Purpose (Dosage, indications, side effects, etc.).
- Familiarity with contemporary medication use systems (interpret prescribers’ orders, prepare and dispense medications, etc.).
- Excellent interpersonal communication skills.
- Systematic problem-solving skills and the ability to work independently with minimal supervision.
- Successfully completes initial and continued competencies as required for maintenance of standard of practice, legal regulatory compliance, and OHSU policies.
Preferred Qualifications
- Board Certification in Ambulatory Care Pharmacy, Pharmacotherapy, or Psychiatric Pharmacy.
- Experience in clinic-based practice.
- Oregon State Board of Pharmacy Preceptor License.
