Position Summary
This is a remote position that requires 50-75% travel in North Chicago and surrounding areas to meet with members face to face. (Must have reliable transportation).
Facilitate appropriate healthcare outcomes for waiver/LTSS members by providing care coordination, support and education for members through the use of care management tools and resources. Provide comprehensive healthcare management services to facilitate appropriate healthcare treatment, effectively manage healthcare costs and improve healthcare program/operational efficiency involving clinical issues. Determine medical necessity/appropriateness, facilitate optimal outcomes, identify and follow through with continuous quality/compliance opportunities. May also include identification of aberrances and initiation of corrective action. Educate/empower customers to ensure compliance, satisfaction and promote patient advocacy. Optimize total costs, implement and evaluate policy based on usage and program directives. Educate/empower colleagues at all levels to enable decision-making at the most appropriate level.
Required Qualifications
- Minimum of 2 years of experience in behavioral health or social services field.
- Minimum 2 years of clinical experience.
- Willing and able to travel up to 50-75% of their time to meet with members face to face in Northside Chicago and surrounding areas.
- Reliable transportation required; mileage is reimbursed per our company expense reimbursement policy.
- Active and unrestricted RN license in IL state.
Preferred Qualifications
- Case management and discharge planning experience.
- Managed Care experience.
- Microsoft Office experience.
- Experience working with HIV and AIDS population.
Education:
- Associate's degree required.
Pay Range
The typical pay range for this role is: $63,200.80 - $136,600.00. This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
