Hospice Nurse Case Manager - Full Time
We are currently seeking a dedicated and compassionate Hospice Nurse Case Manager to join our team. This role is critical as it involves providing end-of-life care to patients in a respectful and clinically effective manner. As a Hospice Nurse Case Manager, you will manage a team and coordinate with other healthcare professionals to ensure that each patient receives the highest standard of compassionate care. This position requires someone who can balance emotional support with rigorous health care standards, ensuring that patient dignity is maintained at all times.

Essential Duties and Responsibilities:
- Oversee the comprehensive care of terminally ill patients to ensure they receive optimal end-of-life care both medically and emotionally.
- Schedule patient visits according to healthcare needs, balancing workload effectively among team members.
- Work in collaboration with physicians to review and manage patient prescriptions and orders to ensure accurate treatment plans.
- Supervise and train LPNs, LVNs, and Hospice Aides, ensuring adherence to all protocols and quality standards.
- Provide emotional support and practical guidance to patients, family members, and other caregivers.
- Participate in interdisciplinary team meetings to discuss care plans and progress and make adjustments as necessary.
- Ensure compliance with all federal, state, and local legal and professional requirements maintaining patient confidentiality at all times.
- Manage patient documentation and records with accuracy and punctuality.
Minimum Position Qualifications:
- Valid Registered Nurse license required.
- Minimum of one year experience in nursing.
- Experience in hospice or palliative care settings preferred.
- Must possess a reliable means of transportation and have a valid driver's license.
- Ability to serve on-call as required by the organization's operational needs.
- Demonstrated ability to manage and supervise a healthcare team.
Preferred Qualifications:
- Associate’s degree or higher in Nursing.
- Strong organizational skills and the ability to multitask effectively.
- Exceptional communication and interpersonal skills.
- Previous experience in a managerial or supervisory role within a hospice care setting.
- Knowledge of electronic medical records (EMR) systems.
Skills:
- Excellent time management and leadership skills.
- Empathy and the ability to handle emotionally challenging situations compassionately.
- Advanced knowledge of end-of-life care processes and best practices.
- Skilled in medication management and palliation.
