This role will be boots on the ground, serving our patients while coordinating with internal St. Jude support partners to ensure a seamless continuity of care from hospital to home.

St. Jude Children's Research Hospital is in search of Home Health Registered Nurse. The Home Health Registered Nurse administers skilled, quality nursing care to patients on an intermittent basis in their place of residence. This position coordinates and directs the home health patient's care based on individual patient needs. This requires the use of advanced pediatric assessment, teaching and decision-making skills. This is performed in accordance with physician orders and plan of care under the direction of the Administrator or designee.
The Home Health RN provides comprehensive assessment, planning, implementation and evaluation for a caseload of home health patients in accordance with the plan of care. Makes the initial evaluation visit and regularly reevaluates the patient's nursing needs by assessing physical, functional, psychosocial and cognitive status of the home health patient utilizing interview observations and physical exam techniques. Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames for applicable patients. Based on evaluations, determines the amount and type of nursing needed by each individual patient. Establishes individualized, measurable goals in consultation with the patient, family and other health care providers.
RN Case Manager position for SJ Housing and/or Community. Typical hours are 0730-4pm. On-Call and holiday coverage required.
Minimum Education and/or Training:
- Graduate from an accredited school of nursing. RN required. BSN preferred.
Minimum Experience:
- Four (4) years of nursing experience required. Pediatric experience preferred.
Licensure, Registration and/or Certification Required by Law:
- (LC: RN-TN) Must possess a current Tennessee State Board of Nursing license if primary residence is Tennessee or a Nurse Licensure non-Compact state.
- If operating a motor vehicle for St. Jude business, must have a valid driver's license from the employee's state of residence and a valid personal insurance policy on the vehicle which meets the requirements for the state of Tennessee.
Compensation
In recognition of certain U.S. state and municipal pay transparency laws, St. Jude is including a reasonable estimate of the compensation range for this role. This is an estimate offered in good faith and a specific salary offer takes into account factors that are considered in making compensation decisions including but not limited to skill sets, experience and training, licensure and certifications, and other business and organizational needs. It is not typical for an individual to be hired at or near the top of the salary range and compensation decisions are dependent on the facts and circumstances of each case. A reasonable estimate of the current salary range is $70,720 - $126,880 per year for the role of Home Health RN.
