Case Manager RN / LIC Full-Time
St. Mary's is a 229-bed, acute care hospital located in Enid, OK. St. Mary's Regional Medical Center has served the healthcare needs of northwest Oklahoma for more than 100 years. We host more than 125 highly skilled physicians offering specialties in Cardiology, Neurology, Orthopedics, Emergency Medicine, Rehabilitation, Laboratory, Women's Imaging, Wound Care, and more.
Position Summary:
Functions as patient and family advocate through initial and ongoing assessment of the patient's needs during hospitalization, identifying and coordinating community resources to meet continuing care needs. Thus, facilitating the patient's movement through the hospital system, with assurance that their needs and concerns will be addressed.
Job Duties/Responsibilities:
- The Case Manager (C.M.) may meet with the patient and family prior to admission or at the time of admission, identifying continuing care issues, instructing the patient and assuring them that their concerns will be addressed.
- The C.M. identifies groups of patients and barriers/delays within the system that contribute to the high cost of patient care.
- The C.M. performs pre-admission/admission screening to determine the needs of patients, their appropriate level of care, 3rd party payer requirements and to initiate discharge planning assessment/education for quality continuum of care needs.
- The C.M., through collaboration with the health care team, develops alternative patterns of care that will provide cost effective care without compromising quality.

Qualifications
Requirements:
- Current RN licensure in the State of Oklahoma required.
- Two years' experience in discharge planning and utilization review desirable.
- Knowledge of QI helpful.
- Excellent public relations skills required as well as communication with physicians, hospital staff, patients, families and community.
- Basic computer skills required.
