Family Nurse Practitioner - Primary Care
We are excited to offer the opportunity for dedicated and compassionate Family Nurse Practitioners to join our Primary Care practices in the beautiful Finger Lakes region of New York State, which includes Dundee, Seneca Falls, and Penn Yan. This direct-hire position involves providing-high quality primary care by conducting thorough assessments, diagnosing patients, prescribing treatments, and managing overall patient care. This role requires practitioners who are committed to developing long-term relationships with their patients and communities. Compensation: $115,000.00 - $135,000.00
Essential Duties and Responsibilities:
- Provide comprehensive patient assessment, including ordering, performing, and interpreting diagnostic tests.
- Make clinical decisions for routine and emergency care and treatment plans to achieve optimal outcomes.
- Prescribe medications following best practice guidelines and state laws.
- Facilitate patient follow-up to monitor treatment progress and outcomes, revising treatment plans as necessary.
- Educate patients on health maintenance and disease prevention.
- Coordinate with healthcare team members to provide holistic patient care and maintain continuity of healthcare services.
- Participate in community health education as per the practice's community engagement strategy.
Minimum Position Qualifications:
- Board NP Certification.
- Current and valid NYS Nurse Practitioner License.
- Minimum of 2 years of experience in primary care or related fields.
- Demonstrated ability to work both independently and collaboratively within a team environment.
- Exceptional interpersonal and communication skills, with a commitment to providing compassionate care.
- Knowledgeable in the use of medical technologies and software for patient care and records management.
Preferred Qualifications:
- Experience in a rural healthcare setting.
- Advanced training or specialization in areas such as pediatrics, geriatrics, or women’s health.
- Leadership experience, including roles in clinical supervision or healthcare administration.
- Familiarity with electronic health records (EHR) systems specific to primary care settings.
- Demonstrated ability to handle a high patient load while maintaining high standards of care.
- Ability to contribute to and lead community health initiatives.
