Posting Summary
Rutgers, The State University of New Jersey, is seeking a Public Health Representative II for the department of Pediatrics at the Robert Wood Johnson Medical School. Provides case management for patients of the Robert Wood Johnson AIDS Program (RWJAP). Adheres to service standard for medical case management-includes assessment and evaluation of patient's short and long-term needs in relation to the HIV status. Provides assistance to patient/family with health care and social service system navigation for the procurement of services.
Among the key duties of this position are the following:
- Completes case management patient needs assessment forms in electronic medical record at outpatient visits.
- Identifies patient/family needs on an ongoing basis.
- Documents patient needs and plan of care in RWJAP patient chart.
- Performs financial screening and obtains paperwork as required by Ryan White grants.
- Obtains "community resources" for patients for health care insurance, charity care, legal services, food, clothing, housing, utility services, drug and alcohol services, educational/vocational services and any other type of service needed.
- Provides written and verbal communication with patient/family regarding any unmet needs and the process to obtain services.

Qualifications
Minimum Education and Experience:
- Bachelor's Degree in a relevant field.
- Two (2) years of professional experience in a health program which shall have included responsibilities for interviewing, investigating, and/or conducting public health surveys or patient care services.
- Must have experience in the collection and handling of environmental and biologic samples.
- Equivalent experience, education and/or training may be substituted for the education requirements.
Required Knowledge, Skills, and Abilities:
- Demonstrated computer skills with Microsoft Office Suite.
Preferred Qualifications:
- Prior experience in other infectious diseases helpful.
- Bilingual in Spanish.
