Job Description:
Assess the diabetes education needs of patients and their support persons. Develop and implement an individualized education plan with the patient and care team. Provide diabetes education to patients, support persons, providers and staff. Support the diabetes education program and provide leadership in care improvement for patients with diabetes.
Principle Responsibilities
- Perform assessment.
- Collect and prioritize data in a process that involves the patient, family, other health care providers, and environment.
- Integrate data to identify patient's diabetes education needs, readiness, barriers to learning, preferred learning style, and readiness for behavior change.
- May assess patient vital signs or other clinical measurements.
- May order lab tests or adjust medications per protocols.
- Analyze data and determine patient needs relative to age, development stage and/or culture.
- Develop a care plan.
- Determine, with patient and family, expected patient outcomes based on assessment and analysis.
- Develop an individualized plan focusing on knowledge and skill acquisition for safe diabetes management.
- Implement the identified plan and evaluates patient progress.
- Implement plan using adult learning principles, behavior change theories, and educational materials consistent with patient's age, health literacy level, and culture.
- Provide diabetes education to patients and their families or support persons.
- Evaluate the patient and family understanding of and response to the plan of care, and adjust plan as needed.
- Document assessment, plan of care, interventions and patient outcomes.
- Support diabetes education needs in the facility.
- Participate in quality improvement activities related to diabetes management.
- Incorporate quality data and current best practices to improve quality and efficiency of patient diabetes management and outcomes.
- Maintain, develop or revise educational literature and maintain equipment.
- May assess and refer patients for advanced self-management, preventative services, or social services.
- May design and facilitate educational offerings for providers and staff.
- May assist in data collection and program planning for department diabetes program.
- May assist with patient scheduling.
- May facilitate diabetes support groups
- May provide diabetes education at community events.
- Other duties as assigned.
Preferred Qualifications
- Bachelor's degree in Registered Nursing
- 2 to 5 years in clinical care and experience with adults with diabetes
- 2 to 5 years in adult education
- 2 to 5 years in documentation in electronic medical record
Licenses/Certifications
- Licensed Registered Nurse - MN Board of Nursing required for Registered Nurses in Minnesota practice settings upon hire or
- Licensed Registered Nurse - WI Dept of Safety & Professional Services required for Registered Nurses in Wisconsin practice settings upon hire and
- Certified Diabetes Educator - National Certification Board for Diabetes Educators required within 1-1/2 years and
- BLS Tier 1 - Basic Life Support - Multisource required from American Heart Association or Allina Health within 90 Days
