Responsibilities:
- Obtains report at beginning of shift and leaves updates at end of each shift. Responds to beeper in a timely manner during shift.
- Triages calls and makes scheduled and emergent visits to patients in hospice or home care program as appropriate. Uses the nursing process of assessment, diagnosis, implementation, and evaluation to provide physical, psycho-social, and spiritual care to patients and their families. Intervenes with patients and families in crisis situations and acts as patient advocate.
- Performs clinical nursing tasks, including phlebotomy, subcutaneous devices, medication administration, and any other nursing care required by the patient at the time of the visit.
- Develops a plan of care for the patient under the order of a physician; maintains ongoing communication as necessary to revise the plan of care to assure quality hospice/home care.
- Assumes on-call responsibility as scheduled after regular business hours for all home care/home hospice patients. Returns calls within 15 minutes and visits patients as necessary. Completes daily on-call log, recording all calls made and received.
- Instructs caregivers in providing care to patients and evaluates the care they provide. This includes, but is not limited to, pain control procedures, medications, use of equipment, and psycho-social support.
- Orders equipment, supplies, and medications as needed by the patient under the order of a physician and assures removal of same after death or discharge.
- Documents all visits, phone calls, admissions, and changes in orders within 24 hours. Records changes in notes, medication records, and care plans as necessary.
- Completes accurately and submits weekly all itinerary forms and risk-indicator incident sheets as required.
- Coordinates patient transfer to IPU, including ambulance transport, sending report and copy of chart to unit, and notification of physician. Provides copy of medical record and transfer summary as soon as feasible.
Home Health Responsibilities:
- Move admission up if cancellations occur from email list.
- Make high-risk calls and/or visits assigned as determined by management team.
Hospice Responsibilities:
- Monitor portals for home hospice and IPU referrals.
- Respond to hospice referral sources in a timely manner.
- Review hospice referrals for hospice appropriateness as well as symptom management for IPU.
- Input all hospice referrals into Netsmart.
- If accepted or to determine acceptance, speak to patient/family/POA/nursing staff at the referring facility and set up SOC.
