This position may have a signing bonus available; a member of the Recruitment Team will confirm eligibility during the interview process.
Responsibilities:
- Implement programming for residents on a consistent basis.
- Facilitate implementation of the treatment plan.
- Facilitate groups, complete daily paperwork including daily logs, group logs, session notes.
- Crisis intervention.
- Collaborate with providers.
- Monitor residents' medication, refills, document medications and changes, provide safe storage and assist while attending appointments.
Qualifications:
- Grade C02: Case Manager I: HSD/HiSet/GED or Associates degree required.
- Grade C04: Case Manager II: Requires a Bachelor's degree; preferably in the field of psychology or human services.
- 1+ years experience in social services, the substance abuse field and/or working with program's population preferred.
- Current valid US-issued driver's license and ability to provide registered, inspected and insured automobile for work-related purposes, including transporting clients in your own vehicle or a company vehicle.
- All candidates will be subject to a CORI review as outlined in the regulations set by the Massachusetts Executive Office of Health and Human Services and a DCF/CPS (Department of Families Adam Walsh/Child Protective Services background check).
