Overview
Job Summary:
Responsible for providing pharmaceutical care to patients and functioning as the service representative for Pharmacy. Scope of duties includes, for responsible areas, overseeing pharmaceutical care delivery, ensuring pharmacy systems meet customer needs, and managing quality & financial performance.
Essential Functions:
- Responsible for providing pharmaceutical care delivery to patients in assigned areas.
- Develops pharmaceutical care plans, protocols, and care delivery meeting therapeutic goals and improving quality of life for each patient.
- Performs consultations in pharmacokinetic dosing, pharmacotherapy, and parenteral nutrition, and other collaborative drug therapy practice as defined by department / organization policy.
- Documents consult and follow up in medical record according to department / professional standards.
- Participates in medical care rounds, transitional care (discharge planning) rounds, multidisciplinary rounds and critical path development.
- Responsible for pharmacy systems and financial performance for assigned areas.
- Functions as pharmacy service representative to medical, nursing and other disciplines regarding pharmacy services.
Education, Training and Experience:
- Pharm. D, M.S., or B.S. Degree
- One year previous health system experience with emphasis on hospital practice preferred.
- Valid Georgia Pharmacist License
