POSITION PURPOSE:
The Physician Assistant or APRN/Nurse Practitioner works with the Clinician team to provide for the overall and ongoing medical responsibility for a patient's comprehensive care, encompassing preventative care and a full range of acute and chronic conditions including end-of-life as appropriate. The PA/APRN works autonomously within their licensure and/or in collaboration with supervising physician, in accordance with the Minnesota Board of Medical Practice and/or Minnesota Board of Nursing.
REQUIRED PROFESSIONAL QUALIFICATIONS:
- Graduate of an accredited formal Physician Assistant or Nurse Practitioner program. Master's required.
- Current Minnesota PA/APRN license.
- Current NCCPA or AANP certification.
- Approved or eligible for approval by the Minnesota Board of Medical Practice for prescription writing privileges.
- Demonstrated computer skills using Microsoft Office products or similar applications.
- Demonstrated excellent oral and written communication skills.
- Strong organizational skills.
- Demonstrated problem solving skills.
- Ability to work independently.
- Experience with an electronic medical record.
- Current CPR certification.
- Active member in professional association.
PREFERRED QUALIFICATIONS:
Gastroenterology: Minimum 1-year recent GI, Med-Surgical, or SICU experience.
PHYSICAL REQUIREMENTS:
- No lifting requirements are identified but the potential for lifting exists.
- Adequate vision, hearing and tactile motor skills.
- Must be able to speak, read and write English proficiently.
ACCOUNTABILITIES:
- Assess the health status of individuals/families as appropriate.
- A complete health history is obtained and documented.
- Patient's perception of problem is elicited.
- A complete, accurate physical examination is performed and documented.
- Appropriate screening and diagnostic tests are ordered and evaluated.
- A problem list is developed.
- Wellness and risk factors are identified.
- Identify variations from norms and diagnose common health problems and diseases.
Develop and implement plan of care through use of Care Model Process
- Sets goals and plans mutually acceptable to patient and clinician.
- Prescribe variety of modalities to meet wellness and illness needs, including but not limited to medication, nutrition, exercise, and traditional medical treatments.
- Manage wellness and illness needs according to Riverway, HealthPartners and ICSI guidelines.
- Teach and support positive self-care activities including problem solving and selection of treatment alternatives.
- Provide health care counseling and emotional support throughout care.
- Interventions are chosen which will promote the patient's participation in his/her own care to the greatest possible extent.
- Evaluate the process and outcome of management plan and modify the plan as appropriate.
Collaborate and coordinate with other health professionals as appropriate
- Discriminate between situations requiring independent management, collaborative management with the Physician and immediate or further referral.
- Collaborate with Physician when unusual incidents or changes in patient's condition occur.
- Coordinate care involving other healthcare providers, outside vendors or employers to meet the needs of the patient.
- Make appropriate referrals.
- Function as a leader, mentor, and educator for other staff members.
Maintain and improve professional practice
- Provide recommendations for improving care are made.
- Maintain professional competency through continuing education.
- Maintain current certification.
- Incorporate clinical best practice guidelines into patient care.
- Participate in peer review activities.
- Participate in yearly performance evaluation.
- Documentation of care is done in accordance with Riverway and HealthPartners standards.
- Participate in appropriate committees.
- Maintain CPR certification.
- Proficiency in electronic medical record.
- Other activities identified in the supervisory practice agreement referenced above.
