Essential Functions
- Delivery of Patient Care
Assumes responsibility for the coordination and delivery of patient/caregiver care. Conducts nursing assessments to assist in the overall management of patient/caregiver needs. When appropriate, develops, implements, evaluates, and updates the plan of care to ensure that care is uninterrupted and that goals are achieved. Facilitates patient and caregiver independence to the extent possible. Appropriately utilizes VIA Health Partners patient-related protocols, standing orders, and guidelines. Conducts effective patient and caregiver education, allowing time for questions and consultation. Communicates effectively with members of the IDG, patients, caregivers, and other health professionals to ensure continuity of care. Maintains open lines of communication with the supervisor and keeps them apprised of any complex patient/caregiver issues. Applies principles of infection control and follows guidelines for universal precautions in accordance with accepted professional practices. - Interdisciplinary Group
Attends, participates, and may facilitate interdisciplinary group conferences [IDG] and special care conferences. Establishes and maintains effective working relationships with team members. Identifies specific patient/caregiver needs and makes referrals to appropriate team members. - Documentation
Ensures accurate, complete, succinct, and timely clinical documentation in accordance with VIA Health Partners guidelines and the requirements of licensure and certification standards. - Professional and Personal Accountability
Demonstrates competency in knowledge and skills. Maintains VIA Health Partners' professional reputation. Assumes responsibility for professional development and stays abreast of current trends in the healthcare field. Incorporates new information and methods into practice. Maintains current professional licensure, CPR certification, and CEU requirements. Exhibits time management skills and maintains productive and professional relations with external agencies and healthcare providers. - May perform other duties as required
(IPU) Cleans patient rooms as required per patient room cleaning procedure. Ensures all areas of the facility are clean and appropriately stocked with supplies. Keeps the family room clean throughout the day. Ensures proper chemicals are used for cleaning different surfaces and that protective equipment is used when working with chemicals, i.e., gloves, glasses.
Minimum Qualifications
- A Diploma or associate degree in nursing is required; Bachelor's Degree preferred.
- A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.
- Current CPR required; CHPN preferred.
- Minimum 1-year related experience for inpatient units (IPU) and 2 years for other nursing positions. Experience in direct care of patients with life-limiting illnesses required. Related experience may include hospice and palliative care; family medicine; internal medicine; geriatrics, oncology; home health care; medical surgical.
- Computer proficiency is required.
