Job Description
The Behavioral Health Consultant supports Primary Care physicians/Health Home teams in identifying and behaviorally intervening with Health Home enrollees who could benefit from behavioral intervention.
Minimum Requirements
- Master's Degree in Social Work or demonstrated/verified equivalent in combination of education and experience
- Licensed Social Worker - LMSW Minimum of three (3) years experience working in a healthcare-related setting
- Good written and verbal communication skills
- Must be able to read and speak the English language
Preferred
- Clinical Licensed Social Worker - LCSW
