Summary of Job Duties: We are seeking a dynamic and experienced Nurse Educator to join our team in El Dorado, AR. This role will work closely with our ICHI projects to evaluate and assess health risk on program participants and patients seen on our mobile health units or community-based locations. Additionally, the Nurse Educator will be responsible for managing relationships with program participants and local organizations. The successful candidate will track program progress and timeline deliverables to ensure the goals and objectives of the project are met, while also ensuring that relationships with participants and institutional stakeholders are maintained throughout the project. They will engage in various activities such as outreach and participant recruitment, participant screening and assessment, health education, care coordination/service navigation, and community education/engagement. The Nurse Educator will play a vital role in tracking program deliverables through effective utilization of program management software and will proactively identify any barriers and ensure that all program objectives are met and data collection is accurate.
Qualifications:
- Graduate of an accredited Nursing Program with an associate degree with seven (7) years of experience as an educator/instructor or RN experience OR BSN in Nursing plus five (5) years experience as an educator/instructor or RN.
- One (1) year of experience in a clinical environment.
- Valid Arkansas Registered Nursing license.
- Current BLS certification OR must successfully obtain certification within the first thirty (30) days of employment.
- A valid AR driver's license is required with current automobile insurance and a reliable car for moderate travel to LR occasionally and to surrounding communities (mileage reimbursed at the current state mileage reimbursement rate) if a state vehicle is not available-occasional out-of-state travel.
- A personal working cell phone is required.
- Must be able to work evenings and weekends as needed.

Key Responsibilities:
Evaluation and Assessment of Health Risk:
- Conduct comprehensive health screenings and assessments for program participants.
- Evaluate health risks and identify appropriate interventions.
- Collaborate with the project team to develop individualized care plans.
- Monitor and evaluate the effectiveness of interventions and their impact on health outcomes.
Relationship Management:
- Develop and maintain strong relationships with program participants and local organizations.
- Provide education and support to program participants regarding their health needs.
- Collaborate with community-based organizations to ensure comprehensive care.
- Address any concerns or issues that may arise with program participants or organizations.
Program Progress and Timeline Delivery:
- Track program progress and identify areas in need of improvement.
- Monitor and report on project deliverables to ensure timely completion.
- Work closely with project team members to ensure adherence to project timelines.
- Identify and address any barriers to program implementation.
Engagement and Outreach:
- Conduct outreach activities to recruit program participants.
- Collaborate with community organizations and events to promote program services.
- Provide health education sessions to program participants and community groups.
- Participate in community events and fairs to raise awareness about healthcare resources.
Care Coordination and Service Navigation:
- Connect program participants with necessary healthcare services and resources.
- Provide care coordination and support to ensure continuity of care.
- Assist participants with accessing and navigating healthcare systems.
- Collaborate with healthcare providers and other team members to address barriers to care.
Data Collection and Reporting:
- Collect accurate and timely data related to program outcomes and participants' progress.
- Utilize program management software to track and report on program deliverables.
- Prepare reports and updates for project team members and other stakeholders.
- Identify trends and make recommendations for program improvement.
