Essential Job Duties:
The RN Case Manager works under the guidance of the Director of Case Management. The RN Case Manager collaborates with other healthcare team members to coordinate and facilitate the effective delivery of quality healthcare services to Palmdale Regional Medical Center patients within the limits of available resources. As a member of the interdisciplinary team, the RN Case Manager focuses on the achievement of positive outcomes for the patient, the provider, the facility and the payer. The RN Case Manager is knowledgeable about and responsible for utilization review, discharge planning and case management of Palmdale Regional Medical Center's patients regardless of the patient's condition, religion, ethnic background or payer.
QualificationsRequirements:
- Ability to read, analyze and interpret general periodicals, professional journals, technical procedures and governmental regulations. Ability to effectively present information and respond to questions from managers, patients, and the general public.
- Ability to apply basic mathematical concepts to practical situations.
- Reasoning Ability
- Ability to define problems, collect data, establish facts, and draw conclusions. Ability to interpret an extensive variety of technical or clinical instructions in mathematical or diagram form, and deal with several abstract and concrete variables.
- Minimum three years of acute care clinical experience. Minimum of two years clinical case management or utilization review experience preferred. Demonstrated accountability, motivation for self-direction and ability to work in a team environment.
- Registered Nurse. Bachelor's degree preferred.
- Current RN California license. Current BLS. CPI is required within 30 days of hire.
