This is a Knowledge, Skill, Ability (KSA) position. This position requires knowledge of and/or extensive experience with the African American/Black community. Knowledge of the traditions, values, and customs of the African American/Black community being served would demonstrate this. This position exists to provide case management services for children and adults with developmental disabilities. The state of Office of Seniors and People with disabilities contracts with Multnomah County to provide case management services. The purpose of case management is to access, coordinate, and ensure the delivery of services and support required by individuals with developmental disabilities. Case management will assist eligible individuals under their individualized plan in gaining access to needed medical, social, educational, and other services.
As the Case Manager 2 KSA African American/Black Community, other duties will include:
Perform complex case management duties for clients with intellectual and/or developmental disabilities. This includes evaluating and documenting whether they are eligible for services and benefits for all federal and state programs in accordance with state, federal, and local rules, standards, and guidelines.
Ensure resources are being appropriately used to meet clients’ intellectual and/or developmental disability needs.
Keep records of cases and write down what was done in line with federal, state, and local rules. Make, send, and change the paperwork needed to make payments and program benefits work better.
Develop, facilitate & coordinate resources for clients and their families
Ensure that clients and their families are aware of the services they are eligible for and entitled to.
Help people and families choose the right place and care plan for their loved one. They can also help them with personal and emotional problems, money planning, and how to interact with others.
Assess the level of functioning and level of support needed of the individual by completing the TXIX Waiver Forms and submitting them to the state for approval
Review level of functioning and level of supports needed by updating the TXIX Waivers on an annual basis
Create and finish Individual Service Plans. These plans are completed at staffing and special meetings in client/families's homes as well as in provider settings of clients with funded vocational or/or residential services.

Minimum Qualifications/Transferable Skills*:
Education/Training:
Knowledge, Skills, Ability (KSA) in the African American/Black Community expertise, knowledge, skills, ability of traditions, values, and customs
Bachelor's degree from an accredited college or university with major course work in social work, psychology/sociology, criminal justice or a related field. (Professional work at or near the level of this position can substitute year after year for the educational requirement.)
Three (3) years of increasingly responsible research, field investigatory or case management experience as a journey-level social services, court, criminal justice or corrections/protective services professional
