Position Summary (Centralized Refill Program and the Hypertension Remote Management Program)
As part of the Centralized Refill Pharmacy team, the clinical pharmacist is responsible for supporting prescription refill renewal authorization for MaineHealth Medical Group patients, using established institutional guidelines and embedded protocols. Using evidence-based medicine, the pharmacist will enhance patient outcomes through appropriate medication management and will triage any additional needed information to facilitate the timely process of refill requests for primary care clinic patients. This position will foster interdisciplinary collaboration with providers, pharmacists, and medical assistants to optimize medication therapy.
Required Minimum Knowledge, Skills, and Abilities (KSAs)
- Education: Bachelor of Science in Pharmacy Degree from a recognized/accredited School/college of Pharmacy required. Doctor of Pharmacy (PharmD) Degree or Master's Degree in Hospital Pharmacy or Business Administration preferred.
- License/Certifications: Current applicable state(s) license with Board of Pharmacy required.
- Experience: Three years of experience in compounding preferred in a large academic medical center preferred. General or specialized pharmacy residency preferred.
