THE POSITION

Wayne County Area Agency on Aging (AAA) has a rewarding position for a patient, compassionate, and organized individual to act as an advocate for older adults. Our goal is to provide support for seniors that will allow them to stay active and safe, while remaining in the community setting of their choice. DESCRIPTION OF WORK

As an Aging Care Manager 2, you will be working with older adults, their families, and professionals through ongoing care management responsibilities. This role includes specialized duties, to include but not limited to protective services, care transitions, and person-centered counseling. Maintaining a caseload of consumers and involvement in necessary areas of assistance will be essential. You may have the opportunity to regularly coordinate services with physicians, community agencies, and financial institutions. Work Schedule and Additional Information:

Full-time employment.
Work hours are 8:00 am to 4:30 pm, Monday - Friday, with 60-minute lunch.
On-call hours when required.
Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

Six months as an Aging Care Manager 1 or a County Caseworker 1; or
Successful completion of the County Social Casework Intern program; or
Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
An equivalent combination of experience and training.

Other Requirements:

You must meet the PA residency requirement.
Hiring preference for this vacancy may be given to candidates who live within Lackawanna, Monroe, Pike, Susquehanna, or Wayne counties.
You must be able to perform essential job functions.

Legal Requirements:

A conditional offer of employment will require a drug screening.
You must pass a background investigation.
This position falls under the provisions of the Child Protective Services Law.
