Disability Support Specialist
Location: Lake Country Area
Creative Solutions of Wisconsin is looking for a full-time Disability Support Specialist. This position is based in the Lake Country area within our growing business. Our company headquarters is located in Southeastern Wisconsin, but our team predominantly works remotely to support clients throughout the state. The ideal candidate will excel in combining professional experience with the tenacity for research to develop customized support plans. Daily workflows include managing assigned cases, capturing all details with appropriate systems, and communicating with the Disability Support Team to meet or exceed goals.
The Disability Support Specialist responsibilities include:
- Work directly with clients with various disabilities and their families through telephone, in person, email, and video conferencing.
- Build reliable and healthy professional relationships with clients that support project progress and trust for the company.
- Maintain authorizations and all supporting documentation needed to ensure billable hours are current and team support of client needs.
- Effectively manage your own time in day-to-day habits to meet daily billable goals.
- Update and maintain accurate and professional records in our project management software daily.
- Track client goals and submit timely reports as required.
- Stay up to date on DHS and other program policies to provide excellent resource solutions to each client’s specific needs.
- Utilize Creative Solutions process in conjunction with providing creative solution support.
- Effectively coordinate and communicate, both verbally and in writing, with large teams of professionals supporting clients.
- Provide case-specific solutions by "thinking outside the box" to find alternative solutions for each problem.
- Inspire others with a positive attitude.
- Build an authentic connection with respect for different types of people.
Required for Success:
- Experience working with the disability community (Social worker, Special Education Teacher, Long Term Care Case Manager, Advocate, etc.) - Min 3 years
- Self-motivated/autonomous
- Adaptable to change
- Likeable/easy to talk to
- Excellent experience with online solutions including but not limited to email, government and agency systems, project management, etc.
- Strong, professional written communication with excellent follow-up skills
- Thrive in a "Think on your feet" environment
- Professional experience using de-escalation strategies
Benefits and Compensation:
This full-time position is compensated at an hourly rate of $35 - $38 per hour, with higher level licensure eligible for compensation up to $43 per hour.
