St. Luke’s Treasure Valley home health team of skilled professionals provides comprehensive health care in the warmth and comfort of the patient’s home. From newborn to seniors, we offer the latest home health techniques and technology to people at all ages and stages of life. Dedicated to the continuum of care as patients transition from inpatient to home to outpatient settings for care. Our home-based services team (NP, RN, MA, MSW) also assists with oversighting transitions. Close working relationship with St. Luke’s Hospice team.
You will support our Home Health and Hospital at Home services.
As a Home Care RN, you will provide comprehensive care to patients with a new diagnosis, medications, or have been discharged from ER, require respiratory treatment or wound care. During home visits, you will ensure patients are progressing with their care plan, provide education related to their disease process, and assess physical and psychological needs to improve quality of life. When it comes time to discharge, you will coordinate resources and educate patient and patient's family.
What you can expect from this role:
- Full Time Days (Fri - Mon | 10 hour shifts) may include rotating evening call
- Weekend Premium Pay
- Applicable Differential/On-call Pay
- Daily visit requirements below state and national average
- Pay for drive time
- Mileage reimbursement (Geographical Area: Treasure Valley - Ada, Boise, Canyon, Gem, Owyhee, Payette, Washington)

Qualifications:
- Education: ASN or BSN Degree
- Experience: 1 year of RN experience; Hospice or Acute Care experience helpful
- Licenses/Certifications: Current RN License in the State of Practice; Current Basic Life Support (BLS) Certification
- Valid Driver's License and proof of Auto Insurance
