Overview
The hospice and palliative care nurse assists patients with terminal or life-limiting conditions and their families with pain, symptom, and plan of care management in their home setting through the department of Homecare. The hospice and palliative care nurse is a critical member of our close-knit, interdisciplinary team, using a wholistic approach in supporting the patient and their family. This position is hybrid post-orientation period. Must practice professional nursing and promotes patient advocacy in accordance with ANA Code of Ethics and maintains a current Ohio Nursing License.
0.8 FTE, 32 hours per week

Responsibilities
- Performs home-based care with family-centered approach.
- Acts as the on-call nurse for a portion of each month, divided among the nursing team.
- Practices with a competent and compassionate approach to supporting patients and families facing terminal and life-limiting conditions.
- Actively participates in the interdisciplinary team and maintains their primary patients' plan of care.
Qualifications
- BSN Required.
- Ohio Driver's License.
- Must have excellent critical thinking skills, good communication and customer service skills.
- Ability to organize and manage time well. Ability to work independently, as well as within an interdisciplinary team.
- Other necessary skills include, but are not limited to, pediatric assessment, infusion and central line care, injections, wound care and patient education. Must be comfortable with end-of-life discussions and patient experiences.
