Position Overview: We are seeking a dedicated and compassionate Primary Care Nurse Practitioner (NP) to join our team at a skilled nursing facility in Hagerstown, MD. The ideal candidate will provide high-quality, patient-centered care to our residents, focusing on their medical needs and overall well-being.
Key Responsibilities:
- Conduct comprehensive assessments and evaluations of residents.
- Develop and implement individualized care plans in collaboration with the healthcare team.
- Manage acute and chronic medical conditions.
- Perform routine physical exams and health screenings.
- Prescribe medications and treatments as necessary.
- Educate residents and families on health management and wellness.
- Collaborate with interdisciplinary teams to enhance care delivery.
- Maintain accurate and timely documentation in electronic health records.
Qualifications:
- Experience in primary care, geriatrics, or skilled nursing preferred.
- Strong clinical skills and the ability to work independently.
- Excellent communication and interpersonal skills.

Compensation:
- Extremely competitive salary of $110,000 to $140,000.
