Description:
The Clinical Manager will be responsible for ensuring the delivery of high-quality patient care, coordinating clinical services, and supervising a team of healthcare professionals. The ideal candidate will have strong leadership skills, clinical expertise, and a passion for improving patient outcomes in home health and group home settings.
Reporting Relationship:
Reports to President/Owner.
Responsibilities/Activities:
- Manage and supervise a team of nurses, therapists, and other clinical staff members for the home health agency and group home.
- Oversee the development, implementation, and updating of patient care plans with ordering physician/practitioner and healthcare providers.
- Conduct regular clinical case conferences and team meetings to review patient progress and coordinate care.
- Monitor clinical outcomes and quality metrics to identify areas for improvement and implement corrective actions as needed.
- Provide clinical guidance and support to staff, including training, supervision, mentoring, and performance evaluations.
- Coordinate with other departments, such as scheduling and billing, to ensure efficient and effective operations, including making patient and personnel assignments.
- Collaborate with community resources and healthcare providers to promote continuity of care and enhance patient outcomes.
- Ensure compliance with all regulatory requirements and agency policies.
- Participate in agency planning, development, and quality improvement initiatives.
- Maintain accurate and complete clinical records using applicable systems.
Required Knowledge and Experience:
- Registered Nurse (RN) license in Indiana with no restrictions.
- Bachelor's degree in Nursing (BSN) required; Master's degree in Nursing preferred.
- Minimum of 5 years of clinical experience, preferably in home health or a related field.
- Minimum of 1 year of clinical supervisory experience in a healthcare setting.
- Solid understanding of Medicare/Medicaid regulations and home health accreditation standards.
Required Skills/Abilities:
- Proven leadership experience with the ability to manage and motivate a team.
- Excellent communication and interpersonal skills.
- Proficiency in using electronic medical records (EMR) and other healthcare technology systems.
Physical and Mental Demands:
- Ability to lift and move patients as needed.
- Comfortable with home visits in various environments and conditions.
- Mental fortitude and stability to handle stress.
- Physical and mental ability to drive a vehicle.
Qualifications/Education:
- Valid Registered Nurse (RN) license in Indiana.
- Bachelor's degree in Nursing (BSN) required; Master's degree in Nursing preferred.
- Certification in Home Health Care Nursing (HHCN) or a related specialty preferred.
