RN Case Manager Home Care - Part Time

We Need:
We have an immediate opening for a Part-time Registered Nurse to do in-home care supervisory visits and initial assessments. The great part of this position is its flexibility and paid per visit.
Responsibilities:
- Each home care nurse is given a monthly list of 10-20 visits to complete during the month.
- This position allows for work in the field and requires regular weekly travel to client homes throughout WAKE County and surrounding areas.
- The registered nurse plans, organizes, and directs home care services with an emphasis on community health education and experience.
- Regularly re-evaluates patient nursing needs.
- Provides health care instructions to the patient as appropriate per assessment and plan of care.
- Performs supervisory visits every 90 days (60 days if CAP) for clients in assigned areas.
- Communicates any changes in client condition with the primary nurse.
- Communicates any concerns regarding aides with the field supervisor.
Requirements:
- A current license as a Registered Nurse in North Carolina or have a multi-state license.
- Prior home care experience strongly preferred, but not required.
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
- Demonstrated ability to read, write, and effectively communicate in English.
- Ability to work independently and manage time effectively.
- Must be able to work as part of a team.
- Strong interpersonal skills.
- Solid computer skills: prior experience with electronic medical records (EMR) preferred.
- Must have knowledge of how to use electronics.
- Must have own reliable vehicle.
- Must be flexible.
- Ability to travel to cases as assigned.
