The Broward County Board of County Commissioners is seeking qualified candidates for Mental Health Specialist (Community Care Team - CCT) with the Broward Addiction Recovery Division.
General Description
Performs a variety of limited technical health, mental health, and/or behavioral health related services within a treatment team for the care of patients. Works under close to general supervision according to set procedures, but determines how or when to complete tasks.
Minimum Education and Experience Requirements
Requires one (1) year working with the substance abuse/mental health population providing support services. Depending upon area of assignment, may be required to have previously been a primary consumer of mental health or behavioral health services.
Special Requirements
Must be able to work flexible schedule including mornings, evenings, weekends, and holidays. May be required to conduct home and community visits related to consumer or client needs.
Special Certifications and Licenses Required
Must obtain Certified Recovery Peer Specialist (CPRS) certification through the Florida Certification Board within 12 months of hire. Required to have and maintain a valid Florida driver's license and authorization to drive on County business.
Preferences
- Associates degree or higher in related field
- Certified Recovery Peer Specialist (CRPS) through the Florida Certification Board
- Certified Addiction Counselor (CAC) Certification or other applicable Florida Certification Board Certifications
- Certified trainer in Whole Health Action Management and/or Worldwide Responsible Accredited Production (WHAM or WRAP)
- Experience being on-call for urgent matters
- Experience with in-home/on-site care
- Bilingual
DUTIES AND RESPONSIBILITIES
The functions listed below are those that represent the majority of the time spent working in this class. Management may assign additional functions related to the type of work of the job as necessary.
PROVIDING SUPPORT AS PART OF A CCT TEAM:
Provide support that affect a person's overall well-being, such as primary physical health care, housing, and social connectedness. Engage the individual in their current setting, such as the crisis stabilization unit (CSU), State Mental Health Treatment Facility (SMHTF), homeless shelter, detoxification unit, at their home, etc. Utilize a standardized level of care tool and assessments to identify service needs and choice of the individual served. Provide frequent contact within the first 30 days of services, which could consist of visits three times or more per week. Serve as single point of accountability for the coordination of an individual's care with all involved parties. Provide rotational 24/7 on-call availability. Develop a care plan with the individual based on shared decision-making that emphasizes self-management, recovery and wellness. Coordinate care across systems, to include behavioral and primary health care as well as other services and supports that impact the social determinants of health. Assess the individual for eligibility of Supplemental Security Income (SSI), Social Security Disability Insurance (SSDI), Veteran's Administration benefits, housing benefits, and public benefits, and assist them in obtaining eligible benefits. For individuals who require medications, ensure linkage to psychiatric services within 7 days of discharge from higher levels of care. Submit documentation of services provided.
PROVIDING SUPPORT IN GENERAL:
Provide emotional, informational, instrumental, and affiliation types of support. Strengthen client confidence in obtaining recovery by providing mutual emotional supports (empathy, caring, concern). Provide clients with a sense of belonging by educating clients on life skills, social skills, prosocial skills, and recreational skills. Empower clients by providing concrete assistance on accomplishing tasks. Assist clients in creating a sense of community by facilitating contacts with community resources, including providing transportation. Observe and engage with clients going during the Baker Act or Marchman Act. Provide a living example to client of how to get from treatment to recovery. Provide supportive availability to others with shared experience. Submit documentation of services provided as directed.
ADDITIONAL JOB RESPONSIBILITIES MAY INCLUDE:
Observe and record random urinalysis when needed. Bridge the service gap between Professional Services (Clinicians/Nurses) and the services of those with lived experience. Monitor the behavior of patients and records pertinent behavioral progress in medical records. Prepare reports, patient records and other documents as required, using computer-based applications. Maintain patient confidentiality as regulated under public health laws, standards, federal confidentiality laws and state regulations. Accompany/transport patients to and from clinics and hospitals and recreation therapy activities. Assist patients to build formal and informal support networks. Assist with the program admission and discharge process. Conduct periodic rounds of patient areas; Prompt patients to participate in scheduled daily program activities. Perform patient personal possession searches. Model self-advocacy and adaptive mediation techniques; Provide supportive interventions. Participate in team treatment planning and implementation. Assist in carrying out individualized care, service, or treatment plan objectives. Assist patients with activities of daily living and skill building, as needed. Performs related work as assigned.
