As a Case Manager, you'll help provide a holistic, patient-centered approach to care that addresses both immediate and long-term needs. Primary Responsibilities:
- Identify, coordinate and/or provide appropriate levels of care under the direct supervision of an RN or MD
- Perform clinical operations and medical management activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring, and evaluating). This includes case management, coordination of care and medical management consulting
- Provide health education, coaching and treatment decision support for members
Expect to travel about 50% of the time to meet with members in the hospital.

Required Qualifications:
- High School diploma or GED or equivalent work experience
- Unrestricted LPN or LVN license in the State where the position will be performed
- 3+ years case management or acute care experience
- Basic computer proficiency (Microsoft Word, Outlook and internet)
- Ability to travel 30-50% of the time to local hospitals on a daily basis
Preferred Qualifications:
- Experience with EMR (Electronic Medical Records)
- Inpatient Hospital Case management experience
- Bilingual - English/Spanish
California Residents Only: The hourly range for this role is $19.47 to $38.08 per hour. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
