Position Overview:
The TFK Case Manager (CM) position supports families that have received early childhood behavioral health consultation. The CM works in partnership with the Early Childhood Clinician to support the implementation of the service delivery plan. The primary responsibility of the CM is to help the family identify and access community supports (e.g., mental health treatment, public school referrals, after-school programs, summer camps, etc.) that will support the family at home and/or help the child access needed services. Upon conclusion of the consultation, the CM will assist the Early Childhood Clinician in obtaining outcome measures and surveys.
Responsibilities:
- Build strong, functional working relationships with families who have received early childhood consultation.
- Work closely with the Early Childhood Clinician and the family to implement, monitor, and modify the service delivery plan as needed.
- Assist in creating and maintaining a positive and collaborative team environment.
- Maintain an ongoing list of community-based supports for young children and their families.
- Assist the Early Childhood Clinician with the distribution and collection of outcome measures.
- Deliver activities at child outpatient clinics, childcare programs, and remotely.
- Complete qualifying activities, progress notes, service delivery plans, communication, and referrals in required program timeframes.
- Participate in all meetings, training, evaluations, supervision, quality management, and related activities as directed by the Clinical Manager/Director.
- Collaborate with the Clinical Manager/Director to set up a working schedule that meets client and childcare program needs.
- Perform other related duties as assigned.
- Comply with established division and program policies, procedures, and objectives.
- Attend a variety of meetings, conferences, and trainings as required or directed.
- Demonstrate use of quality improvement in daily operations.
- Comply with all health and safety regulations and requirements.
- Respect diverse views and approaches and contribute to maintaining professionalism, tolerance, civility, and acceptance.
- Maintain regular, reliable, and predictable attendance.
- Maintain confidentiality and comply with all Federal, State, and local laws and regulations.
Qualifications:
- Grade C02: Case Manager I: Requires a high school diploma plus at least 3-5 years working with young children and families.
- Grade C04: Case Manager II: Requires a bachelor's degree; preferably in psychology, social work, early childhood education, or a related field plus at least 1 year working with young children and families.
- Must have a current, valid, US-issued driver's license and a registered, inspected, and insured automobile for work-related purposes.
- Must be able to pass a CORI background check.
