Position Summary::
Reporting to the Director, Care Management, the Sr. Manager, Care Management provides leadership and direction to drive the delivery of high quality and cost-effective care coordination and services for an assigned population served within Careforth’s Structured Family Caregiving program. These services focus on consumer and caregiver engagement as well as clinical and quality outcomes. In collaboration with the Director, Care Management, the Sr. Manager supports the implementation and ongoing evaluation of clinical programs and care coordination models for an assigned geography. Additionally, the Sr. Manager is responsible for the management of departmental budgets and supporting expected fiscal performance as influenced by care management processes and practices. The Sr. Manager ensures compliance with all contractual, regulatory and NCQA accreditation requirements and promotes a culture of continuous quality improvement, cost reduction outcomes and improvement in the overall quality and delivery of the care management program. This position will have oversight of both the RI and CT Teams.
What You Will Do::
- Oversee the delivery of care management programs, including management and oversight of the interdisciplinary care management teams and care management model for an assigned geography.
- Implement and ensure compliance with policies, procedures and other protocols that guide the consistent and quality delivery of clinical services for all populations served
- Ensure compliance with internal and external regulatory requirements and professional practice standards toward consumer and caregiver assessment, care planning, interventions and ongoing evaluation of consumer and caregiver outcomes
- Oversee quality and performance improvement initiatives associated with program evaluation or mandated by regulatory and contract requirements for an assigned geography
- Monitor trends and develop metrics to identify and prioritize opportunities to improve clinical, quality and fiscal outcomes; develop solutions and mitigation plans in collaboration with leadership team
- Lead, coach and mentor staff, promoting an environment of high performance and engagement, growth, and collaboration
- Perform other projects and duties as assigned
What You Will Bring::
- Bachelor’s degree or equivalent, preferably in a health care or social services discipline is required along with a minimum of 7+ years of progressive experience in care/disease management, working with complex populations, specifically older adults and/or individuals with disabilities
- 5+ years of management experience
- Comprehensive knowledge/experience in managing Medicaid services, particularly LTSS services preferred
- An active RN license is strongly preferred. Additional state-specific qualifications may be required.
- Ability to travel (usually by automobile) within assigned geographic area; valid driver’s license and automobile insurance is required
- Strong working knowledge of case management systems, community service and delivery systems, as well as Microsoft Office suite.
