The Job
The purpose of the Behavioral Health Specialist, Sr. (Lead) will provide leadership support to the Behavioral Health team and Embrace clinical team, guiding them through complex cases while maintaining a clinical focus. This position will be clinical and administrative.
You Will
- Provide short-term mental health treatment to members including assessment, diagnosis, treatment planning, interventions (individual and group), and discharge/aftercare planning.
- Ensure and evaluate clinical appropriateness of care plan by incorporating assessment findings against evidence-based guidelines, clinical reasoning, clinical practice guidelines and/or best practices in the community.
- Serve as the clinical subject matter expert (SME), providing support to all behavioral health team members, including other specialists, ensuring consistent, high-quality care for members.
- Monitor team performance, including tracking referrals, ensuring members’ timely access to appointments, and evaluating clinical outcomes to maintain high standards of care.
- Provide clinical oversight, mentorship, and guidance to the Behavioral Health team, ensuring adherence to best practices, evidence-based care, and seamless collaboration across disciplines.
- Foster team cohesion by supporting in day-to-day program functions and facilitating smooth communication between the BH team and leadership.
- Serve as the key point of contact for Embrace complex behavioral health cases (psychiatric inpatient or skilled nursing), collaborating with all Embrace teams to assist with care coordination.
- Actively contribute to continuous quality improvement efforts and identify opportunities to enhance the effectiveness of BH services and program workflows.
- Triage incoming program referrals, coordinating with team members to ensure timely and appropriate intervention for mental health needs, both internally and with external providers, as necessary.
- Maintain confidential information by following all HIPAA regulations, policy and procedures, compliance, and regulatory directives and by completing required documentation in a timely, professional and accurate manner.
- Prioritize assignments, provide assistance, schedule work, but do NOT directly supervise.
- Coaching and motivational interviewing.
- Actively support the achievement of SCAN’s Vision and Goals.
- Other duties as assigned.
Your Qualifications
- Master's Degree required.
- LCSW required.
- 2-5 years Behavioral and/or Mental Health Experience required.
- 3+ Year post-Licensure Clinical Practice preferred.
- Ability to build strong rapport.
