Overview: Nexus HR is looking for a Nurse Practitioner for a reputable FQHC primary care facility in Inglewood, CA.
About the Job
This position is responsible for providing primary care services, including assessing and managing patients in the clinic following established standards and practices.
Duties and Responsibilities:
- Performs a complete physical exam, obtains complete medical history and records findings, orders appropriate laboratory and diagnostic procedures, interprets and integrates data to determine preliminary diagnosis and therapeutic plan.
- Collaborate with CMO to facilitate Quality Improvement quarterly meetings.
- Present periodic and scheduled reports on clinical care data and related quality improvement data.
- Champions process improvement and PDSA approach in developing, implementing, and revising Quality Improvement initiatives.
- Administers medications and injections in compliance with state law and clinic guidelines.
- Instructs patient/family regarding medications and treatments, educates patients regarding health and illness prevention, and recommends community resources to meet patient and family needs.
- Collaborates with physicians in managing acute and long-term medical needs of patients.
- Provides monitoring and continuity of care for patients between provider's visits.
- Maintains and reviews patient records, charts, and other pertinent information, and posts tests and examination results.
- Triages patient telephone calls and provides consultation.
- Orders, interprets, and evaluates diagnostic tests to identify and assess patient's clinical problems and healthcare needs.
- Records physical findings, and formulates plan and diagnosis based on patient's condition. All documentation should be done in the patient chart (EHR).
- Prescribes drugs or other forms of treatment (within their licensing capabilities).
- Maintains patient confidentiality.
- Attend and participate in team meetings and huddle(s).
- Collaborate in developing team priorities, patient goals, and care plans.
- Collaborates with other care team members through ongoing communication to discuss roles, responsibilities and patient hand-off to enhance the care provided to patients and ensure team-based approach to care.
Qualifications:
- 2 years experience as a Nurse Practitioner.
- At least 3 years experience as a Registered Nurse.
- Masters of Science in Nursing.
- Successful completion of an approved nurse practitioner program.
- California Registered Nurse license.
- Nurse Practitioner license.
- DEA.
- Controlled Substance Registration.
- CPR.
- Hepatitis B Vaccination is highly preferred.
- Able to work in Inglewood, CA.
