Short Description
Linkage to care point person for newly identified HIV-infected persons enrolling in Cooper EIP medical care services. Re-engage HIV-infected persons lost to care. Provide partner services to new and existing HIV-infected patients. Work collaboratively with the medical care team providing patient counseling for management of medical treatment care plans.
Employ social networking strategies with new and existing HIV-infected patients to bring additional social contacts in for HIV testing. Provide prevention counseling/strategies such as CLEAR, ARTAS, or Partners in Health for patients and their families.
This position's role is key in the implementation of multi-disciplinary care.
Experience Required
Minimum of 1 year experience in providing HIV prevention or care services to people living with HIV. Experience counseling HIV (+) clients on connecting to available services. Knowledgeable of community resources.
Education Requirements
BSN required, current NJ RN License.
License/Certification Requirements
Current NJ-RN License, BSN required.
Special Requirements
Division of HIV, STD, and TB Services (DHSTS) clinical navigator training (will be provided).
