Job Description
- Conduct comprehensive diagnostic evaluations and assessments of patients' health conditions
- Develop and implement individualized treatment plans for patients, including prescribing medications and therapies
- Perform medical procedures such as spinal taps and wound care as needed
- Provide health coaching and education to patients and their families, promoting healthy lifestyle choices
- Collaborate with other healthcare professionals to ensure coordinated and comprehensive patient care
- Maintain accurate and up-to-date medical documentation, including coding for billing purposes
- Monitor patient progress, adjust treatment plans as necessary, and provide ongoing support
Qualifications
- Minimum of 1 year of experience as a Nurse Practitioner in a primary care or similar setting
- Minimum of 2 years of experience as a Physician Assistant in a primary care or similar setting
- Experience in senior care is highly preferred
- Proficiency in medical documentation and coding
Skills:
- Strong knowledge of primary care practices and procedures
- Ability to perform diagnostic evaluations and develop treatment plans
- Experience with senior care, including knowledge of common age-related health conditions and treatments
- Excellent communication skills, both verbal and written
- Ability to provide health coaching and education to patients
- Proficiency in medical documentation and coding

Compensation | Min: $122,000 Max: $147,000
Pay range is based on several factors and may vary in addition to a full range of medical, financial, and/or other benefits. Final salary and offer will be determined by the applicant’s background, experience, skills, internal equity, and alignment with geographical market data.
