As a Clinical Manager,Functions as a role model in area of clinical specialty, demonstrating competency in clinical judgment, selected technical skills and appropriate knowledge base to lead and guide staff. Demonstrates clinical expertise in the execution of the nursing process and directs staff in its application to promote safe, effective, and appropriate care. You will work with your teams Case Mangers, Social Workers, Chaplains, and CNAs, as well as our Hospice Physicians, to ensurethat staff is familiar with and adheres to HOC policies and procedures.We empowerour Clinical Managers to serve as a resource to your staff on care continuity and quality.
Main Responsibilities:
Functions as a role model in area of clinical specialty, demonstrating competency in clinical judgement, selected technical skills and appropriate knowledge base to lead and guide staff. Demonstrates clinical expertise in the execution of the nursing process and directs staff in its application to promote safe, effective, and appropriate care.
Hires, supervises, evaluates, and mentors staff assigned to their team, including creating and overseeing a plan of professional development for each staff member to ensure up-to-date credentials and compliance with certification and licensure requirements. Prepares and delivers staff evaluations in a timely manner. Establishes and oversees completion of individual performance improvement plans as necessary.
Conducts patient visits to offer support and expertise to the nursing staff, patients and families. Serves as a resource, ensuring that patient problems are addressed and resolved in a timely manner.
Evaluates the quality of patient care through direct staff observation and review of nursing documentation, review of medical records and patient feedback.
Ensures that staff understands and exhibits the hospice philosophy of palliative care through education and role modeling.
Other duties as assigned by the VP of Hospice Operationsor the Chief Operating Officer.

Your Qualifications:
Bachelors degree in nursing, healthcare administration or behavioral health required. Graduate of an accredited nursing school required.
3-5 years nursing experience in a nursing home, assisted living center, or other post-acute, long term care, or similar facility required.
2-3 years supervisory experience required.
Excellent oral, written and interpersonal communication skills.
Knowledge of Medicare, Medicaid, private insurance and other community programs and resources.
Demonstrated ability to work effectively on an inter-disciplinary team.
Excellent customer service philosophy and skills required.
Understanding of and concurrence with the hospice philosophy of palliative care.
Basic computer skills, including but not limited to Microsoft Word and Outlook, are required.
