Job Title:
Manager-Case Management RN - Coordinated Care-UH
Job Summary:
Functions as a direct supervisor for daily tasks for an assigned team of Case Managers. Functions as an experienced professional expert clinician facilitating the coordination of managers to include employee selection, performance reviews, scheduling, team outcomes management, data collection and reports. Under the direction of the Director of Access Resource Management addresses activities with utilization reviews, case coordination, discharge planning and patient placement. Facilitates effective resource utilization of assigned managers with the identification and removal of barriers to provide efficient team productivity.
Education & Experience:
Bachelor's degree in Nursing, Master’s preferred, and five (5) years of RN experience, to include two (2) years of experience in Case Management, Utilization Review, Utilization Management, or Discharge Planning and two (2) years of supervisory experience.
CERTIFICATIONS, LICENSES OR REGISTRATION REQUIRED:
Mississippi RN license, CPUM (Certified Professional in Utilization Management) or CCM (Certified Case Manager) preferred.
Knowledge, Skills & Abilities:
Responsibilities:
- Determine staffing needs, establish qualification and skills description, and perform hiring activities in conjunction with administration and others as appropriate. Coordinates activities by scheduling work assignments, establishing priorities, and directing the work of case managers.
- Identifies key competencies for case management staff and develops program to assess these competencies from the hiring process to ongoing performance evaluations. Evaluates and verifies employee performance through review of work assignments and techniques.
- Performs daily supervision and assigned duties to include, hiring, determining workload and delegating assignments, training monitoring, counseling, evaluating performance and data collection and reports.
- Provides coaching to managers for further professional development of department. Provides work instructions and assists employees with difficult or unusual assignments.
- Documents findings, plan and actions taken according to departmental guidelines and standards, track and collect data as needed to evaluate benchmark and metric goals.
- Ensures the efficient resource utilization of assigned by identifying and removing barriers that impact efficient team productivity. Serves as a resource for resolving difficult patient placement.
- Performs ongoing staff development activities in conjunction with the department director. Leads and participates in staff meetings. Enhances job related performance by continuing education and related job knowledge.
- Provides back-up coverage for case managers to ensure departmental goals are met.

Physical and Environmental Demands:
Requires occasional exposure to bio-hazardous conditions such as risk of radiation exposure, blood borne pathogens, fumes or airborne particles, and/or toxic or caustic chemicals which mandate attention to safety considerations, occasional working hours significantly beyond regularly scheduled hours, occasional activities subject to significant volume changes of a seasonal/clinical nature, occasional bending, occasional lifting and carrying up to 10 pounds, occasional crouching/stooping, occasional kneeling, occasional pushing/pulling, occasional reaching, frequent sitting, frequent standing, occasional twisting, and constant walking.
