Job Summary and Qualifications
The Manager of Case Management Services is responsible for promoting patient-centered care by coordinating all aspects of hospital-based case management for their assigned area of responsibility in alignment with the goals of the department.
What you will do in this role:
- Serves as a role model and functions as a clinical resource for staff.
- Supervises and facilitates development of staff and oversees the day-to-day tasks/activities in the department.
- Ensures staff are competent in the use of InterQual to progress the plan of care.
- Works collaboratively and maintains active communication with physicians, nurses, and other members of the interdisciplinary care team to effect timely, appropriate patient management.
- Monitors the quality of CM interactions with patients, families, physicians, and the multidisciplinary team by regularly shadowing staff to ensure clear understanding and adherence to department processes.
- Monitors quality of documentation and assessments by regularly auditing the case managers’ documentation.
- Participates in clinical performance improvement activities. Communicates effectively with patients, family, medical staff, care team members, students, supervisors, and outside contacts.
- Assumes responsibility for scheduling, staffing, education, and competencies of department.
- Completes daily, weekly and monthly reports as assigned by the Director of Case Management.
- Seeks ways to control costs without compromising patient safety, quality of care, or the services delivered.
- Adheres to established policy and procedure and standards of care; escalates issues promptly through the established chain of command.
- Demonstrates knowledge of regulatory requirements, HCA Ethics and Compliance policies, and quality initiatives.
- Serves as an advocate for patient's rights, needs, and values; ensures that patients’ ethnic, cultural, or religious values, beliefs, preferences, and needs are considered and aligned.
What qualifications you will need:
- Associate Degree in Nursing or Nursing Diploma required, Bachelor's Degree in Nursing preferred.
- Current licensure as a Registered Nurse in the state of practice required.
- 3-5 years of clinical nursing experience required.
- 2 years of supervisory or management experience preferred.
- InterQual experience preferred.
- Certification in Case Management and/or InterQual or IQCI preferred.
